Flex Plan Form
NEW FOR FALL 2024! Faculty have the ability to complete a Flex Plan for students. The Flex Plan outlines how this accommodation will be facilitated during the current semester. Once submitted, a copy of the Flex Plan will be submitted to OA for approval and then sent to the student.
Attendance Consideration
The Attendance Accommodation exists to provide justification for absences related to a student’s documented disability or health condition. In most cases these absences do not require a visit to the Medical Clinic or a doctor’s office. The student has been instructed to communicate with faculty in a timely manner when they need to use this accommodation. The Attendance Accommodation is not meant to exempt a student from an instructor’s attendance policy.

Steps to Complete Flex Plan Form

Log into AIM Faculty Portal.

Agree to Access Policy.

Click Flex Plan under the Home menu.

Click View, this will open the Flex Plan Form.

Complete the List of Questions.

Upload Syllabus. Instructors have the option to upload a syllabus to better help facilitate the accommodation based on your course policies.

Instructors have the option to copy the form to the same course or to other courses. Select the option that applies: Other Requests in the Same Coure or Other Requests in My Courses.

Under Form Submission, check I agree to the statement listed and click Initiate and Request Review.

The student's assigned Accommodation Specialist will review the form and send it to the student.
