OA Testing Information Form
When a student requests to use testing accommodations in your class, they can be provided by the instructor or OA. The preferable method is for the instructor or GTA to administer the test with the given accommodations. This allows the student the same access to instructors or GTAs that their peers have when questions pertaining to the test arise. This also allows immediate access to the test for grading purposes.
If it is determined that an instructor is unable to proctor a student with testing accommodations due to scheduling issues or the student’s specific accommodations requested, the OA Testing Center will be glad to proctor the exam. To start this process, instructors will need to complete an OA Testing Information Form.
Steps to Complete OA Testing Information Form

Log into the AIM Faculty Portal.

Agree to Access Policy.

Click on Alternative Testing under the Home menu.

Review the Important Message and click OA Testing Information Form in the tool bar.

Scroll down the page to find your courses. Click Specify OA Testing Information Form for each course you need assistance from OA.

Coplete the List of Questions with informaton of how your exams are administered in your course.

NEW FEATURE: Instructors can now specify the dates of their exams, quizzes, and final with a new calendar feature.

Select the type Exam, Final, or Quiz.

Use calendar for Date.

Enter Time of exam, final, or quiz.

Enter Standard Length of Exam in minutes. *Do NOT include extended time. AIM will calculate this for the student based on their accommodation.

Provide Phone Number in case OA Staff needs to contact you. *Students will not see your phone number.

Click Submit and Continue to Specify Exam Dates.

On the next page, you can continue to Add Additional Exam Dates or Modify dates of exams you have already submitted.
